Pro Tour Active Members are those bowlers who intend to participate on the Pro Tour. Pro Tour Associate Members are those who do not at this time intend to bowl on the Pro Tour, but who would receive all other benefits of Active Members, including receipt of all the official correspondence and results. The following must be completed by Pro Tour Active Applicants unless the average requirements were met on the prior year's pro tour.

Note on starting times: Preferred starting times for this tournament and the following five events will be awarded to bowler s in the order in which they have submitted their paid applications. It should be noted that starting times can only be determined after we know the number of entries, and in applying for Tour Membership, the bowler accepts that fact that he may be required to bowl at a time other than his first preferred time. You will be mailed a confirmation of starting time along with directions.
Entry Fees: Ladies - $350.00 & $55.00 Membership Fee - $135.00 is required with application, $135.00 prior to second tournament and the final $135.00 prior to the third tournament. "Charter Members" receive a $25 discount on the final payment.

I have read the accompanying "Official S.P.B.C.A. Rules for Entry to Pro Tour and Tournament Conduct" which I have in my possession, and agree to abide by these rules.
